Who speaks for you? Liberty Hospital to host free event May 2 for National Healthcare Decision Day

LIBERTY, MO (April 12, 2017) – Liberty Hospital will host a free opportunity May 2 to complete legal forms in recognition of National Healthcare Decision Day. Notaries will be available to help participants complete a durable power of attorney for healthcare form.

“Accidents happen every day, and families are forced into situations they never are prepared for and decisions must be made without knowing the true wishes of even their closest loved ones,” Liberty Hospital Risk Management Director Joanie Peterson, MSN, J.D., CHC, said. “These conversations are not easy, but are necessary. Once you have these conversations, it’s important to fill out a durable power of attorney for healthcare form that states your specific wishes given extraordinary circumstances.”

A notarized durable power of attorney for healthcare form is a legal document that expresses an individual’s healthcare wishes when he/she is unable to do so. In this document, an individual appoints a person to serve as his/her primary healthcare agent to make any necessary care or treatment decisions. This person then is responsible for communicating those wishes to your attending physicians and care providers.

This is a free community service open to the public. Notaries and members of Liberty Hospital’s Risk Management Department will be available from 7:00 a.m. to 6:00 p.m. in the lobby at the main entrance of Liberty Hospital, 2525 Glenn Hendren Drive, Liberty. No appointments are necessary. To print a form in advance of the May 2 event, go to www.libertyhospital.org and click on patients/visitors, then advance directive forms. Forms also will be available at the event.
